Dear Parents/Guardians,

This page has links above for all forms that the athlete and parent to need to sign and submit. The first three packets of forms are required for an athlete to participate in any interscholastic athletic sports program that OAHS offers.

Before each sports season starts, there is a parent meeting to go over policy and procedures with the athletic director, business manager, and athletic trainer. Afterwards, parents meet with their student's coach to go over their team's policies, procedures, game times, practice times, and who can help drive to away games.

There is also a donation letter for parents and for businesses to donate to a particular sport or they may donate directly to the Athletic General fund that is used by all sport teams.

**Parents, if you want to help drive any of our student athletes to away games, there is a MANDATORY Parent Driver Packet (above) that must be filled out and turned into the Front Office to Mrs. Domingues.

Please click on and download each packet to fill out and turn in or you may stop by the Front Office and pick one up.

You may not drive until you have confirmed with Mrs. Domingues that you have been approved to drive, so please check in with her first before driving any of the student athletes.

Attention Football Parents: There are 3 additional forms that are only for football players, this has to do with tackle insurance, please make sure that your health insurance covers tackle football (see paperwork for requirements). For these forms goto football's page and click on forms.
